Referral Scheme – Terms & Conditions

General terms and conditions of the Centre for Dentistry Referral Scheme.

1. Introduction
1.1 These terms and conditions apply to the Centre for Dentistry Referral Scheme.
1.2 The Centre for Dentistry Referral Scheme is in place to provide benefits to patients who are advocates of Centre for Dentistry and refer patients to us.
1.3 The Referral Scheme is not available at Oldbury and Christchurch Centre for Dentistry practices but all other practices nationwide are participating in the scheme.

2 About the scheme
2.1 A referral is defined as a person aged 18 or over who is referred to Centre for Dentistry as a new paying patient.
2.2 Referrals must be aged 18 years or over.
2.3 Referrals cannot already be a registered patient of CFD or an employee of CFD.
2.4 When a new patient joins Centre for Dentistry, our standard terms of service apply.
2.5 A referral is deemed complete once the patient has attended and paid for their appointment at the practice.

3. Scheme rewards
3.1 The reward programme is a cumulative scheme. Referral rewards are offered to the referrer after 3, 5 and 10 referrals eg. After 3, a further 2 and a subsequent 5 qualifying referrals.
3.2 Upon receiving successful referrals for 3 new paying patients, Centre for Dentistry will give the referrer a free electric toothbrush. The toothbrush must be claimed within 1 month from the date of first notification.
3.3 Upon receiving successful referrals for 5 new paying patients, Centre for Dentistry will give the referrer a free scale and polish appointment at their registered practice. This scale and polish must be booked and attended within 3 months from the date of first notification.
3.4 Upon receiving successful referrals for 10 new paying patients, Centre for Dentistry will give the referrer a free at home teeth whitening treatment. Treatment must be booked and commence within 3 months from the date of first notification.
3.5 The referrer will be notified by CFD when they are eligible to claim their reward. This is classed as the date of first notification.

4. Duration of the scheme and alterations
CFD reserves the right to vary or make changes to the referral scheme, these terms and conditions, and the benefit levels applicable to each referral at any time.

5. Transferability of referral scheme benefits to other CFD patients or dental surgeries
Whilst CFD’s referral scheme benefits are not transferable to any third party, participants of the referral scheme are entitled to receive their benefits at any of Centre for Dentistry’s dental practices. Should a referral scheme member wish to transfer to another practice a written request must be submitted to CFD for approval.

6. Third party rights
A person who is not a referring patient at Centre for Dentistry has no legal rights under the Contracts (Right of Third Parties) Act 1999 to enforce any of these terms and conditions or the provisions of the Policy Document.

7. Governing Law
These terms and conditions and the policy document shall be governed by and constructed in accordance with the laws of England and Wales and will be subject to the exclusive jurisdiction of the English Courts.
